Our 27 yr old male came to the casuality with the chief complaints of
Fever since 3 days
Body pains since 3 days
Swelling of scrotum since yesterday night.
HOPI:-
Patient was Apparently asymptomatic 3 days back then he developed headache which is insidious in onset , gradually progressive and relieved on taking medication.swelling of scrotum since yesterday night which is sudden in onset gradually progressive associated with pain which is continous .Heaviness & swelling of both lower limbs which is pitting type present upto ankle ,for which he was taken to local hospital. on investigations low platelet count was found,There fore they were referred here.Fever which is low grade not associated with chills & rigors.
Not associated with burning micturition,black coloured stools, giddiness,red colured urine.
PAST HISTORY:-
Not a k/c/o DM &HTN.
Deafness present since birth .
PERSONAL HISTORY:-
Diet - mixed
Appetite- normal
Bowels- regular
Sleep - regular.
Addiction -occasionally alcoholic.
FAMILY HISTORY:-
Not significant.
EXAMINATION:-
Patient was conscious, Aphasic, cooperative well oriented to time,place and person .
Moderately built & well nourished.
No pallor,Icterus,cyanosis, clubbing lymphadenopathy.
Pedal edema present.
Temp-99 f
PR-82 BPM
RR-16 cpm
Bp-110/80mmhg
Cvs- S1,S2 +
R/S- BAE+
P/A- soft,non tender.
CNS-NAD

1.IV fluids 1NS,1RL @ 100ml/hr
2.Tab.DOXY 100 mg /po/OD
3.Tab.DOLO 650mg/po/TID
4.INJ.ZOFER 4 mg /IV/SOS
5.INJ.NEOMOL 2 gm/IV/SOS
6.INJ.OPTINEURON 1 amp in 10 ml NS/IV/OD.
